Here is the summary of the current recommendations for the prevention of Lym=
e Disease in a person who has been bitten by a Black-legged tick - from Upto=
Date - the online and constantly updated medical textbook that nearly all ph=
ysicians who want to stay current, use ----------

B. burgdorferi is not usually transmitted within the first 48 to 72 hours of=
 tick attachment. The likelihood of transmission is increased if the tick is=
 engorged and/or has been attached for at least 72 hours. (See 'Tick attachm=
ent' above.)
Ticks are best removed with tweezers or protected fingers, pulling straight u=
p with even pressure. (See 'Tick removal' above.)
We suggest that patients who meet all criteria for antibiotic prophylaxis be=
 treated (Grade 2B). The criteria for prophylaxis include:
Attached tick identified as an adult or nymphal Ixodes scapularis tick (deer=
 tick)
Tick is estimated to have been attached for =E2=89=A536 hours (by degree of e=
ngorgement or time of exposure)
Prophylaxis is begun within 72 hours of tick removal
Local rate of infection of ticks with B. burgdorferi is =E2=89=A520 percent (=
these rates of infection have been shown to occur in parts of New England, p=
arts of the mid-Atlantic States, parts of Minnesota and Wisconsin)
Doxycycline is not contraindicated (ie, the patient is not <8 years of age, p=
regnant, or lactating) (see 'Approach to prophylaxis' above)
If the patient meets all of the above criteria, the recommended dose of doxy=
cycline is 200 mg for adults and 4 mg/kg up to a maximum dose of 200 mg in c=
hildren =E2=89=A58 years, given as a single dose. If the patient cannot take=
 doxycycline, we do not recommend prophylaxis with an alternate antibiotic. A=
 reasonable alternative for patients who meet criteria for prophylaxis but w=
ho prefer not to take antimicrobial prophylaxis is to observe the tick bite s=
ite and treat for Lyme disease if an erythema migrans (EM) rash develops. (S=
ee 'Approach to prophylaxis' above.)
We recommend that patients who do not meet the criteria for antibiotic proph=
ylaxis be observed and treated only if signs or symptoms of Lyme disease (eg=
, EM) develop (Grade 1A). (See'Approach to prophylaxis' above and "Treatment=
 of Lyme disease".)
We recommend not performing serologic testing for Lyme disease in patients w=
ho report a tick bite. (See 'Serologic testing' above.)
